Heart of Kent - Spring Walk Consent Form

The Spring Walk takes place on the 26th May 2022. In order to participate, all attendees must complete this questionnaire. One form must be completed per person.

7. Confirmation of Booking & Consent I recognise that trail walking is an activity with a danger of personal injury or death. Participants in these activities should be aware of and accept these risks and be responsible for their own actions and involvement. I agree that medical treatment may be given to me if necessary, including the administration of a general anaesthetic and to surgical operations in the case of an emergency, in accordance with the recommendation of a qualified medical practitioner. The customer accepts that outdoor activities take place in varied natural environments where there are natural hazards and risks to manage. Every effort will be made by the Heart of Kent Walking Festival Walk Leaders to ensure an activity is provided in a safe manner. The Heart of Kent Walking Festival has the right to alter the activity if the company believes it necessary due to dangerous and/ or unsuitable conditions for the activity. Any participant who is believed to be under the influence of drugs and/or alcohol will be refused participation in the experience. Although prior experience is not necessary, customers are expected to be of good general health. The medical section must be completed and all prior injuries and/ or serious illness must be declared. The participant must satisfy him/ herself that taking part in an activity/ course is with his/ her own capabilities. The organisers reserves the right to refuse a booking on medical grounds if it is detrimental to the safety and smooth running of the activity. Participants are not insured by the Heart of Kent Walking Festival against personal accidents. The Heart of Kent Walking Festival accepts no responsibility for accidents or injury to participants or for loss of or damage to personal effects, unless caused by the negligence of Maidstone Borough Council, or any member of its staff. The personal information requested by the company is held in its original form and on computer. Contact details will be used to advise you of future offers either by post or email. If you do not want to receive future mailings please advise us. We do not pass on your details to any third party. From time to time photographs taken on Heart of Kent Walking Festival activities may appear in the Heart of Kent Walking Festival and Visit Maidstone website, brochures and promotional material. If customers do not wish to be photographed please raise this with the Heart of Kent Walking Festival Walk Leader at the time. I have received full information and agree to my participation in all outlined activities. If I present with the signs of COVID-19 prior, during or up to 7 days following my experience I will notify the Heart of Kent Walking Festival immediately.
